I hope this letter finds you in good health. I am writing to inform you that after careful consideration, I have made the decision to close my practice on March 16th, 2026. Regrettably, we will no longer be able to continue our doctor-patient relationship. Please know that this was not an easy decision.
I understand that it is very difficult to find a new family doctor. Unfortunately, none of the other doctors at Doctors Medical Clinic can accept new patients. If you require assistance in finding a new physician, I recommend contacting HealthLink at the phone number 811 or online at www.healthlinkbc.ca.
Until the closure date of March 16, 2026, I will continue to provide your care. I will be following up on your test results and reports. Please ensure you contact me before then to refill prescriptions. After March 16th, 2026, be assured that none of your prescriptions, pending consultations or investigations will be canceled. After March 16th, please remind any laboratories, medical imaging locations hospitals and specialist physicians you see that my practice has closed and that other arrangements will have to be made for reports and follow-up. Hopefully you will have found a new family doctor or can make arrangements for tests and reports to go to a walk-in clinic or Urgent Care Center.
Your medical records will be safely stored and distributed with the non-profit association, MedRecords. All medical records will be securely stored and transferred at a minimal cost. I recommend that you obtain a copy of your record from MedRecords or arrange for the transfer of a copy to your new physician by
